Mission to Israel 2024

PARTICIPANT WAIVER AND RELEASE

I voluntarily agree on behalf of myself, and any minors listed below to participate in the Mission to Israel sponsored by Temple Israel of Hollywood on February 14–19, 2024.

I acknowledge and agree that Temple Israel of Hollywood does not undertake or accept any duty or liability for my safety, and I agree to assume any and all risks of bodily injury, death, disability, and property damage as a result of participating in the Mission to Israel, whether such risks are known or unknown to me.

By my signature, I hereby acknowledge and agree that participating in travel and activities involves risks of bodily injury that may include permanent disability or death. I acknowledge and accept all dangers and risks associated with the Mission to Israel, whether or not expressly listed herein. 

By my signature, I hereby waive and release Temple Israel of Hollywood and its directors, officers, employees, volunteers, and other agents from and against any and all claims, demands, actions, causes of action, losses, or liabilities whatsoever arising from or related to my participation in the Mission to Israel. By my signature, I warrant that I am not relying on any oral representations, statements, or inducements apart from the statements made on this form.

By signing below, I confirm that I have read, understand, and consent to the terms of this waiver and release and that I have the authority to sign this waiver and release on my own behalf and on behalf of each of my family members participating in the Mission to Israel.
