Mammoth Shabbat 2023: Participant Waiver and Release

I voluntarily agree on behalf of myself, and any minors listed below, to participate in the Mammoth Shabbat Retreat program sponsored by Temple Israel of Hollywood on February 17 – 20, 2023.

I acknowledge and agree that Temple Israel of Hollywood does not undertake or accept any duty or liability for my safety and I agree to assume any and all risks of bodily injury, death, disability, and property damage as a result of participating in the Mammoth Shabbat Retreat, whether such risks are known or unknown to me.

By my signature, I hereby acknowledge and agree that participating in winter outdoor activities including, without limitation, skiing, snowboarding, walking, snowshoeing/hiking, tubing, travel and other activities, involve risks of bodily injury that may include serious permanent disability or death. I acknowledge and accept all dangers and risks associated with the Mammoth Shabbat Retreat, whether or not expressly listed herein. 

By my signature, I hereby waive and release Temple Israel of Hollywood and its directors, officers, employees, volunteers and other agents from and against any and all claims, demands, actions, causes of action, losses or liabilities whatsoever arising from or related to my participation in the Mammoth Shabbat Retreat. By my signature, I warrant that I am not relying on any oral representations, statements or inducement apart from the statements made on this form.

By signing below, I confirm that I have read, understand, and consent to the terms of this waiver and release, and that I have the authority to sign this waiver and release on my own behalf and on behalf of each of my family members participating in the Mammoth Shabbat Retreat.
